Your Active Duty physical Exam is a two-part process. Part I includes Completion of Forms, Lab Work, Eye Exam, Hearing Exam, EKG (if applicable), and Vital Signs. Part II will be booked 7 – 10 days following the completion of a Part I, and it will only be booked by the Medical Readiness Clinic (No Part II will be booked over the phone). Both Part I and Part II are to be completed on a by appointment basis. Please contact the appointment call center at 301-295-6289 to book a Part I appointment. Requirements to Initiate Part I for the Physical Exam:


  1. Active Duty: You must be currently serving on Active Duty.

  2. TRICARE Prime enrollee to one of the following medical clinics:

    • Internal Medicine - Medical Home Clinic
    • Fairfax Family Health Center (Navy Personnel)

      For TRICARE enrollment, see WRNMMC TRICARE Health Benefits Office. Reservists contact Adelphi Reserve Center, (301) 394-5800.

  3. Type II Annual Dental Exam completed within the past twelve months for Routine Periodic Physicals or within the last six months for Separation or Retirement physicals. For appointment, see the Dental Readiness Clinic.

  4. For Women, the following must be completed within the past twelve months:

    • Well Woman Exam. Make an appointment with your Primary Care Manager. See Appointments.
    • Mammogram (40 years and older). For an appointment, see Breast Care Center.

Prepare for Your Physical:
Bring Medical and Dental Record to the Physical Exam Office.
Fast for 12-14 hours prior to laboratory testing.
Do Not consume any alcohol for 72 hrs prior to laboratory testing.
Contact Lenses need to be removed for the Eye Exam, eye glasses are recommended.

What to expect at your visit:

The physical examination office staff will screen your Medical and Dental records, review the above documentation for individual requirements, assist members with completion of required documents, and provide age appropriate preliminary exam testing. Associated laboratory test will be ordered under your Primary Care Manager and an appointment for Part II will be booked 7 – 10 days after the necessary blood tests have been collected.
