- Survival
Skills
You will
not only understand diabetes but how to live and thrive. Our
expert diabetes specialists cover the disease process, medications,
nutrition, exercise, complications, health maintenance, traveling,
sick day management, and psychosocial aspects.
- One
4 hour program.
- Open
to patients with diabetes, family members, and staff.
- Every
other Tuesday, 8:30 a.m.-12:30 p.m.

- Self-Management
Knowledge
covered in survival skills plus self-help tools will give you
control of your diabetes and help you to restore quality of
life.
- For
TRICARE beneficiaries with Type 1 or Type 2 diabetes.
- Learn
how behavioral goals and modifying your lifestyle will reap
health rewards.
- Three
4 hour sessions (12 hours total).
- Maximum
benefit gained by attending entire program.
- Participants
are followed for three months after the program.

- Pre-Diabetes
Class
Patients
at risk for developing diabetes will learn lifestyle modifications
to prevent the disease: healthy habits, physical activity, and
nutrition.

Diabetes
Case Management
This
is a comprehensive, personalized program that is tailored to your
specific needs. You will be assessed by our team of specialists
who will then help you chart a course to stabilize and control
your diabetes. That is the goal of the program. Along the way,
you will receive education, monitoring, and personal support.
A nurse practitioner coordinates the program with the team that
includes your primary care manager.
Throughout
your participation, your blood sugars, medications, and diet will
be monitored. Using the tools of assessment, goal-setting, behavioral
modification, and self-management, you will ultimately reach your
personal goal.
- Enrollment
- For diabetic
patients willing and able to self-manage their disease.
- Referral
from primary care manager (PCM) required.

Nutrition
Clinic
- Open
to all patients enrolled to WRNMMC with diabetes.
- You
will be referred to a nutritionist by your PCM or the Diabetes
Care Center nurse practitioner.

After a CON order is sent to Nutrition Department, you will be
advised to make an appointment by calling 301.295.4630.

The nutritionist will see you in the Diabetes Care Center.

Clinic offered on Monday afternoons by a registered dietitian/certified
diabetes educator.

Foot
Care Clinic
- Open
to WRNMMC diabetic patients that require routine foot care only.

Referral required from primary care manager (PCM).

Clinic held on Monday and Tuesday morning.
Diabetes
Support Group
- 1st and
3rd Wednesday of every month.
- Hours -
9:00 - 10:00 a.m.
- Location
- Diabetes Care Center Education Room, Bldg 9, 1st floor, next
to Internal Medicine.
- Facilitated
by psychologist.
- Open to
all diabetic patients and family members.
