Diabetes Case Management

Nurse talking to patient. This is a comprehensive, personalized program that is tailored to your specific needs. Along the way, you will receive education, monitoring, and personal support. A nurse practitioner coordinates the program with the team that includes your primary care manager.

Throughout your participation, your blood sugars, medications, and diet will be monitored. Using the tools of assessment, goal-setting, behavioral modification, and self-management, you will ultimately reach your personal goal.
